Opioid Misuse Survey
PanCare of Florida has recently received a Rural Community Planning Grant to assess the opioid response needs of our communities.
In collaboration with county public health, law enforcement, emergency medical services and non-profit community wellness organizations, we have established the Panhandle Rural Opioid Partnership. The partnership’s service area includes Calhoun, Franklin, Gulf, Holmes, Jackson, Liberty, Madison, Taylor, Walton and Washington Counties.
Our first step is to garner community input on current opioid use conditions. We have created a survey open to all who live, work or serve in our partnership service area.
Please join us by completing this survey and sharing it with your community through email, social media accounts, or other web-based services.
Please Note: This survey is anonymous, personal identifying information such as name, address, etc. will not be asked. All information gained in this survey will be used solely to assess the need for services, and gaps in services within our community.
With your help, we can create a plan to best support prevention, treatment and recovery efforts regarding opioid misuse.
